Office Policy
Health History
To ensure a safe, customized session, before your first appointment you will be asked to fill out a Confidential Health History Form. Please use the forms link above to print and fill out these forms before your first appointment. Please either bring these forms with you to your first appointment or email them to the therapist at clinicalmassagetherapies@gmail.com. If you are unable to download the form, please arrive 10-15 minutes early to fill out these forms prior to your appointment. To provide proper therapy at every subsequent appointment, it is the client’s responsibility to always inform the therapist of new and existing health conditions.

Confirmations
Appointments are confirmed 48 hours prior to your scheduled appointment via the email address you provide upon booking your appointment. Please use the link at the bottom of the your email to confirm your appointment. If no confirmation email is received in reply, we will assume you will be keeping your scheduled appointment time. If you need to change your appointment, you may do so by calling the clinic.
Cancellations
We understand that unanticipated events happen occasionally in everyone's life. Business meetings, project deadlines, flight delays, car problems, and illness are just a few reasons why one might consider canceling an appointment. In our desire to be effective and fair to all clients and out of consideration for the therapists' time, we have adopted the following policies:
24 hour advance notice is required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment.
If you are unable to give us 24 hours advance notice you will be charged a 50% cancellation fee. When canceling an appointment, you are more then welcome to schedule for a future date, however this amount must be paid prior to your next scheduled appointment.
No Shows
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a no-show. They will be charged 50% of the service amount for their missed appointment and future service will be denied until payment is made.
Arriving Late
Appointment times have been arranged specifically for you. In respect for my time and yours, please arrive on time for your appointment. If you are late for your appointment, you will be provided as much time as available within the scheduled time slot. You will, however, still be charged the full amount for your alotted time.
Gift Certificates
Gift certificates are available for purchase at the clinic as well as online and make a great gift. They do, however, expire within one year of purchase. We apologize, but gift certificates cannot be replaced if lost or stolen nor are they transferable. The 24 hour notice of cancellation applies to gift certificates as well, although rather than being billed for the appointment, the gift certificate will be considered as having been used. Certificates are not redeemable for cash.
Clinic Etiquette
Please be considerate of others in the clinic by turning off cell phones and pagers when in our office. Unfortunately our office cannot accommodate watching your children during your appointment. Treatments are available for children 17 years of age and under with written parental permission.
Payment
Payment is expected at time of service. CASH, CHECK ($25.00 additional charge if returned), Visa, MasterCard, and Discover are accepted. Fees, discounts, specials, promotions, etc., are subject to change.
Physician Referral
A physician referral is generally not required in order to receive massage therapy. After speaking with your therapist and reviewing health history information, a determination will be made prior to your session if a doctor’s note is required. If you have been referred by a physician, please bring your referral with you. Massage therapy is not a substitute for medical examinations and/or diagnosis. It is recommended that you see a physician for any physical ailment you have.
